Note: To start off, this game is based on the Japanese anime of the same name. So if you are familiar with the cartoon, and let your kids watch it, this game does not add any additional objectional content, and you don't really need to read further. If you have never heard of the cartoon or this game, read on...
Blood: There is mild to moderate blood in this game, but not in actual gameplay. It's only present in some of the cutscenes that are clips from the anime (cartoon), and usually shows up in the form of light splatters/sprays, and it also visibly stains clothes.
Specific scenes of blood:
An early scene shows a side character jump in front of kunai (throwing stars) to protect the main character (who is 12). You see as they hit him in several places, including the leg, which shows some blood soaked into his pants. The enemy then throws an oversized throwing star at the main character, and the side character again protects the main character by stepping in front of him. The oversized throwing star pierces the protectors back. He does eventually recover.
A later scene shows the enemy injure the main character's hand (you see the cuts on his hand), and you soon find out that the weapon used had poison in it. He is told that he has to drain the poison out of the wound, so he takes a knife to the wound (you don't see the impact). After the knife is in, you do see some blood drip onto the floor.
In another scene, an enemy uses sharp, thin knives to throw at one of the main characters, resulting in several of them sticking out of his body before he collapses. The main characters are also visibly bloodied with cuts all over their faces. Right after, one of the main characters is about to finish off the villain, when the villains protege (a young boy), gets in the way and is killed. You see the boy's blood splattered on the villain's face. After this, for complicated reasons, the villain turns on a rival villain. He clamps a knife in his mouth (he lost the use of his arms in the previous fight) and rushes through a group of enemies, slashing and stabbing as he goes (you do see a bit of blood splatter). When he reaches his target, he stabs him in the back. Also worth mentioning is that while he was running through the crowd, they stick him with spears and swords, which are visible protruding from his back.
A later part of the game shows the main villain of the series, who resembles a human snake, and is overall very creepy and disturbing. In this scene, he beats on the main characters pretty badly, apparent in their bloodied appearance. You then see him bite one of the main characters in the neck, resulting in two puncture marks and a bit of blood oozing from the wounds.
Near the very end of the game, the leader of the village, while protecting its citizens, dies by the hand of the aforementioned main villain, by being run through with a sword with bloody results.
One of the main enemies breaks a side character's arm and leg. After the side character is taken to the hospital, the villain sneaks in and tries to finish him off, but is stopped before he can do so.
Suggestive themes: The main character has a ninja technique called "Sexy Jutsu" that makes him look like a very attractive and much older female version of himself; for distracting enemies while fighting. In this form "she" also appears naked (but most often in a one piece bathing suit). However, all of "her" private parts are completely covered by solid clouds enveloping "her" body.
One objective early in the game is to teach a side character "Sexy Jutsu," and test your technique by entering a women's bathhouse while disguised--but you are immediately caught and scolded. It is shown that the main character doesn't quite know what he is doing; in other words, he's still pretty innocent.
You can approach certain male characters in this game and use the "Sexy Jutsu" technique, and if you do, a cutscene will pop up where the male character will go gaga over the somewhat scantily clad form (she's in a pretty modest one-piece) and fall over.
