Note: Keep in mind that this game is over a decade old, and as such, many of the really violent and gory moments listed below are pretty abstract because of the blocky, blurry and outdated graphics. That said, the cutscenes have more overall violence than the actual gameplay.
Animated blood & gore: There is small, almost transparent puffs/clouds of blood when you or the enemy is shot or hit. Blood also lingers and pools on the ground. There is no option to turn blood off in this game.

Animated violence: This game has some bizarre, almost mythological story elements, with villains having almost supernatural powers, from being able to control minds and levitate, to using ravens to mark prey for death. Add all this and a load of complicated politics, theology, and a little conspiracy theory, and you have Metal Gear Solid.
The majority of the in-game violence is limited to killing your enemies with an assortment of guns and grenades to knocking them out or stunning them with stun-grenades and punches/kicks. But it's almost impossible not to kill the enemy because the stunning techniques are short-lived, and even on the easiest difficulty setting, it's pretty hard to stay alive.

Use of Tobacco: The ESRB didn't mention this in their rating. At the beginning of the game, Snake mentioned that he likes to smoke cigarettes. They are also selectable from the inventory, but are seen as a negative, and if selected, they start to slowly suck your health away.
Language: The ESRB didn't mention this in the rating for some reason. there are 27 uses of d**n, 21 uses of h**l, 5 uses of b****rd, and 1 misuse of Jesus.
Closing Comment: This is the game that started one of the best series in gaming history, but overall it's pretty outdated in every way by today's standards. If this game were more detailed graphics-wise, I would recommend it for ages 17+--but since the graphics are pretty abstract, and thus making the violence and gore more mild, I recommend this game for ages 14+.
